When will a substitute parchment be issued?

A substitute parchment may be issued when the original parchment needs alteration in the following cases:

  • name change
  • the original parchment has been issued by predecessors of Queensland University of Technology (including Brisbane College of Advanced Education, Brisbane Kindergarten Teachers' College, Kedron Park Teachers' College, Kelvin Grove Teachers' College, Kelvin Grove College of Teacher Education, Kelvin Grove College of Advanced Education, North Brisbane College of Advanced Education, Queensland Institute of Technology, and the [Queensland] Teachers' Training College).

How to apply

To apply for a substitute of an award parchment you should download and complete a Request for Replacement Parchment form, also available from the Student Centre enquiry counter on your campus.

Alternatively you can apply by writing to Student Services Services including your:

  • full name
  • date of birth
  • student number (if known)
  • mailing address
  • name of course for which certificate is requested
  • campus
  • year study was last undertaken.

The original parchment must be provided with your application. For change of name, supporting documentation such as a marriage certificate or deed poll must accompany the request.

Cost involved and how to pay

There is a fee of $50.00 that can be paid at the cashier at the Student Centre enquiry counter on your campus or alternatively by sending a money order or cheque payable to QUT with your application. Please allow 10 working days for processing.

My original parchment has been lost

If the original has been lost a statutory declaration should be submitted with your application.

If the original parchment has been damaged it should be returned with your application.

Applying from outside Australia if my original parchment has been lost

If applying from outside Australia where a statutory declaration is not available, complete an equivalent declaration attesting to the truth of your statement and witnessed by an officer authorised to witness an oath.

Alternatively make a declaration on a legal firm's letterhead attesting to the truth of your statement and have it witnessed by a solicitor registered in the country from which you are applying.
