When completing the form, please ensure you: • complete Part B only Please print this page and use it as a reference while completing your application form. Step 1Write your full name (first, middle and surname) in the "Applicant’s Name" box at the bottom of each of the four pages of the application. Step 2ONLY COMPLETE PART B OF THE STUDENT BLUE CARD APPLICATION. Parts A and D will be completed by QUT upon receipt of your completed application form. Parts C and E will be completed by a prescribed person (see Part E of the application form for more information). PART B – Volunteer’s Details(Please write the following answers to the corresponding questions you will find on your Blue Card application form) 8. Place a TICK (√) in the appropriate title. 9. Your Present Name refers to the name by which you are currently recognised. Surname refers to your last name/family name and first name refers to your given name.
10. If you have an abbreviation/nickname/alias of your first name, please write it in the space provided. 11. If you have ever been known by any other names other than the name you listed in question 9, write it here. Please place a TICK (√) in the box describing why your name was changed. You must list all names you have been known by from birth. Attach a separate list if required. 12. Place a TICK (√) in the box indicating your gender. 13. Write your date of birth in the format day/month/year (e.g. 22/10/1982). 14. Write the details of your place of birth. 15. The Commission for Children DOES NOT accept addresses outside of Australia, therefore if you do not have a Brisbane address please enter C/- Kathy Grgic, Student Business Services, Queensland University of Technology, Kelvin Grove Campus, Victoria Park Road, Kelvin Grove, QLD 4059 for this question. The Commission for Children will mail your letter and blue card to the above address. An email will be sent to your QUT email address as well as your non-QUT address (if this information has been indicated on your international application form) advising you to contact Student Business Services via telephone upon your arrival to Australia. Arrangements will be made with you on the date and place of pick-up of your Blue Card when you contact Student Business Services. You can organise to pick up your Blue Card from either our Kelvin Grove, or Gardens Pointcampuses. It is preferable that you have already obtained your QUT Student ID card from the Student Centre before you collect your Blue Card. Please note: when you arrive in Australia you will need to contact the Commission for Children and advise them of your postal address. You will also need to notify them when you are leaving Australia. 17. List your current contact telephone numbers. 18. Place a TICK (√) in any boxes that apply to you. 19. PART C – Proof of Identity DocumentsThis section is only to be completed by the prescribed person sighting your identification documents (see Part E of the application form for more information ). Information for Prescribed Person The Commission for Children and Young People and Child Guardian Act 2000 (Australia), requires that any student wishing to complete Professional Experience with children under the age of 18, must undergo a criminal history check and obtain a Blue Card prior to commencing Professional Experience. A “Student blue card application form” must be completed in order to undergo the criminal history check. As this student is located more than 50 kilometers from the QUT campus, the Commission for Children requires that a prescribed person sights the applicant’s original identification as well as verifies the applicant’s signature, full name and date of birth. The prescribed person needs to check that both the signatures that the applicant has provided on the form are the same as the one shown on the original identification. The prescribed person needs to complete the following sections: PART C – Proof of Identity Declaration

PART D - This section will be completed by QUT staff. YOUR APPLICATION IS NOW READY TO BE FORWARDED TO QUT It is extremely important that you DO NOT mail your application form directly to the commission for children. The placement officer from QUT needs to authorise the submission of your personal details to the Commission for screening purposes. Step 3It is important that you keep a copy of your completed application for your records Step 4Mail or courier your original application to: International Student Business Services Once your completed application has been received by the Placement Officer, it will be signed and forwarded to the Commission for Children for processing. When the commission receives your application they will process it and then forward it onto the Queensland Police Service who will check for any criminal history. Depending on the outcome of your criminal history check, if your history is clear then your blue card will be forwarded to the placement officer. You can collect it when you arrive in Australia.
